Ordering the DVD

DVD Order Form Directions

The DVD order form is now able to be filled out electronically and e-mailed. Directions are as follows:

  1. Click on the link above to bring up the pdf.
  2. Fill out the form online and save to your computer.
  3. Click on the e-mail link for Sheryl Kildall and attach the form to the e-mail.
  4. Add any message you wish to the body of the e-mail and click send.

Alternatively, you can print out the form, fill it out, and either fax or postal mail it to us.
